Customers
applying for new mobile connection are mandatorily required to ccomply
following Government of India licensing guidelines
New
Customers
1. You
will need to fill up and sign customer acquisition form (CAF)
2. Paste
your passport size photograph on CAF with cross signature
3. Carry
your original proof of identity and proof of address with you. A copy of the
same will need to be self attested across the document by you and certified by
retailer as original seen and verified.
PROOF OF
IDENTITY ( any one following):
Passport, Arms License, Driving License, Electioncommission ID card, Ration card with photo, CGHS/ECHS card, PAN card, Certification
of address with photo issued by a) MO/MLA/Group A Gazzetted officer on letter
head, b) Govt. educational institutions (for students only), c) Village
panchayat head or its equivalent authority (for rural areas), photo credit
card, address card with photo issued by Dept. of posts, smart card issued by
CSD, Defense / Paramilitary forces, current passbook, of post office or any
scheduled bank having photo, photo ID card of central / state Govt. and PSUs,
caste and domicile certificate with photo issued by state Govt. Pensioner card/
freedom fighter card/ Kisan passbook having photo.
PROOF OF
ADDRESS (any of the following):
Passport,
Arms license, Driving license, Election ID card, Ration card with address,
CGHS/ECHS card, Certificate of address issued by a) MP/ MLA/ Group A Gazzetted
officer on letter head, b) Govt. recognized educational institutions (for
students only), c) village panchayat head or its equivalent authority (for
rural areas), Water bill, telephone bill, electricity bill, credit card statement
(not older than 3 month), Income tax assessment order, vehicle registration
certificate, registered sale/ lease agreement, address card with photo issued
by dept. of posts, current passbook of post office or any scheduled bank, photo
ID card having address ( of central/ state Govt. and PSUs), caste and domicile
certificate with address and photo issued by state Govt. pensioners card,
freedom fighter card, kissan passbook, having address.
