DETAILS TO BE FILLED IN THE FORM:
Know Your Resident (KYR) Infromation required by Aadhaar

• Name - Full name without salutations/ titles, as in Proof Of Identity (POI) document.
• Age / Date of birth
• Gender
• Address - Write full address, as your Aadhaar letter will be deliverd at this address. Plese bring the original Proof Of Address (POA) document
• Corrections / additions to the base address mentioned in the POA document are permissible
• You can include the name of Parent / Guardian / Spouse as part of the address.
• Your mobile number and email ID will facilitate better communication of your Aadhaar number.
• Bank Account - You may choose to open a new Aadhaar enabled bank account / post office saving bank account, or can link your existing account to your Aadhaar number. Relevant details requested by the operator may be provided.
• Consent - Yoy need to specifically express willingness / unwillingness to share the information you provide, with agencies engaged in delivery of welfare services.

KYR + Information:
State government  / banks / India Post might seek additional information relating to social wefare benifits availed by you. please fill in accurate details and carry necessary documentary proof in original.
