DETAILS TO BE FILLED IN THE FORM:
Know Your Resident (KYR) Infromation required by Aadhaar
- Name - Full name without salutations/ titles, as in Proof Of Identity (POI) document.
- Age / Date of birth
- Gender
- Address - Write full address, as your Aadhaar letter will be deliverd at this address. Plese bring the original Proof Of Address (POA) document
- Corrections / additions to the base address mentioned in the POA document are permissible
- You can include the name of Parent / Guardian / Spouse as part of the address.
- Your mobile number and email ID will facilitate better communication of your Aadhaar number.
- Bank Account - You may choose to open a new Aadhaar enabled bank account / post office saving bank account, or can link your existing account to your Aadhaar number. Relevant details requested by the operator may be provided.
- Consent - Yoy need to specifically express willingness / unwillingness to share the information you provide, with agencies engaged in delivery of welfare services.

State government / banks / India Post might seek additional information relating to social wefare benifits availed by you. please fill in accurate details and carry necessary documentary proof in original.

Book Your E-Tickets Through Your Agent Outlet
- IRCTC has a network of about 1,50,000 authorized agent outlets across the country for the convenience of its customers.
- Details of authorized agent outlets are available on the IRCTC website http://www.irctc.co.in/
- You can check the list of agent outlets of your area by entering your area Postal PIN Code.
While visiting an IRCTC agent make sure that: -
- You visit a genuine IRCTC authorized agent outlet - IRCTC authorization letter should be prominently displayed.
- Demand for a requisition form to fill up the details and always ask for a receipt for all your bookings / cancellations.
- Check that the service charges levied by IRCTC are displayed. If not, you may ask for the same.
While Collecting Your E-ticket , Please Check: -
- Check whether you mobile number is printed on the e-ticket. Mobile number is mandatory for all passengers. All agents are required to enter the passengers mobile number.
- Soon after booking the e-ticket , you will receive a SMS from IRCTC on this number with details of your bookings including the fare and service charge.
- Cross check your e-ticket charges with the SMS received from IRCTC.

There are no additional charges for printing of tickets or stationery charges.
- If the agent demands more than the prescribed service charge, you may lodge a complaint in the e-mail ids of agentcomplaint@irctc.co.in or care@irctc.co.in or call us at 011-39340000.
- All e-tickets should have the name of the agent outlet booking the ticket along with address, contact number and name of the Principal Agent.
- E-tickets can be cancelled through the same agent outlet without any additional service charges for cancellation.
- Never Buy Tatkal E-tickets from Agents between 8 AM To 9 AM The Tatkal tickets are made available to the passengers through the IRCTC website http://www.irctc.co.in/ and the railway reservation counter on first come first serve basis.
- Between 8AM To 9AM, the website is open exclusively for individual users who wish to buy Tatkal e-tickets on line as the agents are barred from booking Tatkal tickets during that time.
- If it is found that you are traveling with Tatkal tickets booked by agents between 8AM to 9AM, your ticket is liable to be cancelled.
- Never Book Tickets With Fictitious Names If you are found travelling with tickets under fictitious names, your e-ticket is liable to be cancelled by th Railway Administration and penal action may be initiated.
Note: -
IRCTC Agents are only authorized to book e-tickets (print out on plain paper). I-tickets (provided on Railway Ticket Stationery) cannot be sold by them.
Cancellation Process: -
Before Train Chart Preparation:
- E-Tickets can be cancelled on Internet till Chart preparation of the train.
- Log on to http://www.irctc.co.in/ and go to "Booked Tickets" link and select the ticket to be cancelled and can initiate the cancellation by selecting the passengers to be cancelled.
Cancellation of e-Tickets after the preparation of Reservation charts:
- After chart preparation, users have to file the online Ticket Deposit Receipt (TDR) and track Status of the refund case through tracking service provided by IRCTC. Refund is credited to Customers Bank account after receipt from Railways.
Refund of Wait Listed Tickets: -
- The ticket in which on one PNR, all the passengers are on waitlist, PNRs are dropped from the reservation charts and money is refunded in respective bank account electronically

The new Karnataka Guarantee of Services to Citizens Bill, 2011 was cleared by the cabinet.
purpose:
1. End to corruption
2. improving quality of service delivery with specified time frames.
3. If an official fails to deliver on time, he will be penalized and if he continues to be inefficient, action will be initiated.
INCENTIVE: The incentives for honest officials. Have a certificate of appreciation, and earn up to Rs 5,000 a year. It is to encourage and enhance the efficiency of officers.
If officer rejects the application of a citizen, for he has to record the reasons in writing for doing it. Also, the citizen has two levels of appeal. The designated officers have to render the service. If he fails in his responsibilities, then a citizen can appeal before the competent officer(CO) and get her/his grievances remedied.
If the citizen is unhappy even with the CO, she/he can appeal before the appellate authority. Both the competent officer and the appellate authority have the same powers as are vested in a civil court while trying a suit under the Code of Civil Procedure. They may order the designated officer to provide the service.

As per Revenue department (Bhoomi and urban properties ownership records) the khata has no legal validity because there are multiple khatas issued for a single property.
"The khata does not stand as a record for the ownership of a property."
The government will start up a project there citizens will be issued unique ID cards for their properties. The project to create property records will be taken up in 50 wards of Bangalore . Though there is a mandate to form these records in the Land Revenue Act. Properties will be issued unique identity cards. An urban property ownership records scheme will be launched in Bangalore in January 2012. A record will have the map and exact location of the property, besides ownership details.
Easy registration
The new facility to register any property at any sub-registrar's office. The property could be anywhere in the city or even a faraway village. Changes in khata are on the cards with new measures to ensure the records are genuine."The khata card will be required for all landrelated transactions, and will serve as a unique ID for property, just as the Aadhaar number will serve as a unique ID for the citizen,"
"Revenue officials will survey each and every property. Service centres will be set up for property owners to come to us and submit documents of property ownership,"
During title enquiry, it's possible that more than one person will come forward to claim ownership. "they will listen to every person and scrutinize every document before issuing a unique ID,"

BESCOM introduced regular monthly CIM.
HOW TO USE CIM
1. CIM is a platform for both consumers and local BESCOM officials to interact every month. Consumer can voice there long pending grievances, opinions and suggestions in CIM. The following is the procedure to be followed:
when the consumer attends the CIM for the first time, she/he shall submit a written note on grievances/suggestions/opinion with the reception counter and take an acknowledgment, mentioning a BESCOM ticket number (BTN). In that the address, telephone number and RR number of the consumer shall be mentioned. If these three details are not mentioned, incomplete or incorrect, then the grievance petition is liable for rejection.
2. BESCOM will act upon the said written letter of the consumer within 30days, and will present the action taken in the next meeting through a presentation. The consumer may choose to attend the next meeting and has the choice to disagree with the action taken. In that case, it will be posted to next meeting. Even in the next meeting if issue is not responded to the satisfaction of the consumer, and then will get a personal hearing with the executive engineer (EE) within 2 weeks. If not satisfied at EE's response, then the consumers issue is escalated to the BESCOM corporate office. Grievance will be sent to the independent "Consumer Grievance Redressal Forum" (CGRF). Appeals from CGRF lies before the Ombudsman, office located at KERC premises, mahalaxmi chambers, MG Road, Bangalore.
3. Any communication sent to any other officer of BESCOM, including the corporate office, will be redirected to the local office only, unless the issue is regarding policy etc. the CIM meetings will be invariably videographed to ensure transparency. Also, no consumer will get a chance to talk in the meeting, unless he has submitted the issue in writing in the previous month meeting. The consumers of BESCOM are requested to make use of this new initiative of BESCOM.
